Mail-In Registrations

MAILING IN CLASS REGISTRATION FORMS FOR MDTC MEMBER’S MAIL-IN REGISTRATION

This process is for MDTC Members ONLY who are unable to register for classes online using a credit card.  Please download the appropriate forms below, then mail your completed form(s) with a check/money order, and any discount or priority codes (1 form per class) to

NOTE 1: Assuming student slots are still available, these class applications will be entered manually within business 48hours of receipt and in accordance with the member’s registration priority schedule for that session.

NOTE 2: MDTC Members using this process MUST be sure to include any members status, priority, or other discount/promo codes being used for that class registration.  

NOTE 3: Once the MDTC member’s enrollment has been entered and uploaded, a confirmation will be sent to the email listed on the class registration application.
